If accepted, the student will attend the Arts Elective Showcase.
I affirm that my student is in good standing at his current school and will remain so though the end of the current school year. He/She attends school on a consistent basis (no more than 10 absences this year, arrives to school on time, has had no behavior issues and receives grades of D’s or better.
I am aware of the Arts, Communication and Leadership theme of Rowlett Middle Academy and am committed to fulfilling the school’s parental requirements.
I understand the principal of Rowlett Middle Academy may recommend the revocation of choice if you or the student fails to comply with attendance, punctuality and student conduct requirements.
I understand the expectation to work closely with teachers and school staff to support my child’s academic progress including attendance at all parent/teacher conferences.

A copy of the most recent report card.
